Photo Release:
I grant permission and consent to MI Early Parenting, LLC and The Emerging Parent to use my or my child’s photograph publicly to promote their business. I understand that at times photographs may be taken during classes or play sessions without notice.
I understand that the images under any legal condition, including but not limited to in print publications, online publications, presentations, websites, and social media for publicity, copyright purposes, marketing, illustration, advertising, products, and web content.
I understand that no royalty, fee or other compensation shall become payable to me by reason of such use. I understand that it may not be possible to honor revocation requests due to the nature of online marketing, but that I can ask permission for my photos to be taken down in writing and the request will be taken into consideration. I understand that if I have a child who is in foster care and unable to be photographed, this request will be honored as long as I notify staff upon entry.
I understand photography release is posted for review on site at The Emerging Parent, available when I sign up for any class, event, or session, and is posted on the website.
Waiver:
As a condition of being allowed to enter the premises at The Emerging Parent and MI Early Parenting, LLC or participate in any of the programs of offerings provided by MI Early Parenting, LLC, the undersigned, on his or her behalf, and on the behalf of the participant(s) identified as part of my account acknowledges, appreciates and agrees to the following conditions: I represent that I am the parent or legal guardian of the participant(s) named on my account, or have obtained permission from the parent/legal guardian of the participant(s) named above to execute this agreement on their behalf. I agree that the participant(s) and I shall comply with all stated and customary terms, posted safety signs, rules, and verbal instructions as conditions for participation in any offering or program at the Emerging Parent. In addition, if I observe any hazard during our participation, I will bring it to the attention of the staff on site or official immediately. I understand that I am responsible for my child’s safety and behavior at all times each visit.
I am aware that there are inherent risks associated with participation in activities at The Emerging Parent and on behalf of myself and the participant(s) on my account, I knowingly and freely assume all such risk. both known and unknown. including those that may arise out of the negligence of other participants; I for myself and the participant(s) named on my account. and our respective heirs, assigns. administrators. personal representatives, and next of kin, hereby release and hold harmless, The Emerging Parent, MI Early Parenting, LLC, 2018 Cedar LLC, management. their affiliates, officers, members, agents. employees, other participants, and sponsoring agencies from and against any and all claims, injuries, liabilities or damages arising out of or related to our participation in any and all The Emerging Parent or MI Early Parenting, LLC programs, activities, parties, and the use of the play area.
I understand this waiver is posted for review at The Emerging Parent, on our website. and made available when I sign up for any class, event, or session.
